Dental Appointment Requests

To manage the high volume of requests for dental exam appointments, we are using an online registration form, which is available for the first 2 consecutive business days of each month. Patients interested in being scheduled with one of our dentists should fill out this form.

Our Sliding-fee Plan

We believe everyone deserves access to healthcare, regardless of insurance status or ability to pay. We offer a sliding-fee scale program, called the Healthy Neighbor Plan, that may reduce your cost for care. Depending on your household size and income, you may qualify. All are encouraged to apply.
